New Patients

Please take a moment to fill out the following two forms completely. If you have any difficulties, do not hesitate to call our office for assistance. Please bring a copy of your insurance card to your child's appointment, as well as any prior dental records.



Financial Responsibility
In an effort to reduce the cost to you, the patient, while maintaining the highest level of professional care, we have established the financial policy as follows:

•  Payment of any account balance is due in full within 20 days of statement date.
•  We gladly accept Visa or MasterCard payments.
•  Payment plans are available through CareCredit.

The demand for top quality pediatric dental care has never been higher. We at Hi5 Dental recognize that you have plenty of options to choose from in the area, which is why we strive so hard to provide our patients with the best care possible. Everything we do we strive to be the best at, from offering a warm and inviting environment to our gentle chairside manner we are certain you and your child will have a great first visit.

First Visit - What to Expect

After filling out the new patient registration and medical and dental history forms you will be ready for your first visit to our office. We will start off your child’s appointment with a welcoming introduction and a walkthrough of all procedures to calm their nerves. The first visit will always include a cleaning and exam.

During the examination we will take note of any problem areas in your child’s mouth that may need additional attention, and we will make you and your child aware of such issues. During the examination we will also take an X-ray of your child's mouth. This will help set a baseline for us to compare against in the future. It will also help us spot any issues that may be hidden or hard to see with the naked eye. We use the X-ray to look for tumors, cysts an even bone fractures, although these issues are less prevalent in children, it is still a concern and worth looking into.

Friendly Reminder

We want your first visit to our office to be enjoyable and easy on both you and your child. A great way to introduce a nervous child to our office is to call us to setup a pre-appointment "Happy Visit" During the Happy Visit we will get a chance to meet your child and will take them around the office for a tour. On the tour we will introduce your child to all the different tools and dental technology we use. They will get to meet Dr. Pham and many of our friendly staff.

The day of the appointment we ask that you show up at least fifteen to twenty minutes ahead of time so that you can fill out the proper forms. If your child takes medication, we would ask that you have a list of any prescriptions they take.

On the drive to our office it's not a bad idea to tell positive stories of your past dental visits, or even sing a song just to lighten the mood. We realize that there can be some stress over a dental visit so it's a good idea to keep the mood light and fun on your way to our office.

We know you have a lot of choices in the area when it comes to your child's oral care and that's why we work so hard to make both you and your child happy and healthy. Insurance Information

Dental insurance can be tricky, so if your child has it please feel free to let us help you determine the coverage you have available to you. Because every plan is different it is important to submit any insurance information you have to us well ahead of your scheduled appointment so that we can go over it ahead of time. It would be best to submit any new or changed insurance information to us at least two days ahead of time or, when you submit your child’s new patient paperwork.

If your child ends up needing treatment service, we will be able to give you a breakdown of roughly what your insurance will cover and how much of the procedure will end up being charged directly to you. It is important to note that our professional services are provided to your child, not to the insurance company. The insurance company’s responsibility also is directly to your child. With that in mind, we will do our best to work directly with the insurance company on your child’s behalf while filing any medical or dental claims.

Understanding Dental Insurance

Dental insurance can often be daunting to understand so we find it is important to provide our patients with a breakdown of basic information about most dental insurance plans. Much like health insurance, most dental insurance providers offer a variety of plans that scale to the needs of the consumer. So while you may have the same dental insurance provider as your co-worker your level of coverage may vary depending on a few factors.

Not all dentists participate in the network of, or accept all, dental insurance networks. A list of some of the insurance networks that we accept is located at the bottom of this page. It is important to note that we accept many dental plans and there is a good chance that we just don’t have yours listed. Give us a call at (503) 644-4749 to find out if we accept your dental insurance.

Some of our clients are covered by more than one dental plan. It is important to note that that amounts paid by both dental plans can’t exceed 100% of your total dental expenses. Benefits that apply to your dependents and vary quite a bit from one plan to another. Make certain to note any special clauses that involve dependents on your plan.

Most dental insurance plans are calculated with a “benefit period” in mind. Typically, a benefit period runs the span of one year. Keep in mind when your benefit period starts and ends so that you can take full advantage of your plan before the period ends and your deductible is reset.
